How do you make a focal B&W pic w/ photofiltre that has more than 1 focal point? - t b more condition_symptoms

I have a black and white for the Coordination of me, with my eyes and lips. I know how to link B & W pic w / 1 focus, but when I click on another part of the picture, the highlight disappears. Help?
